For Iron Punch I think you need an Iron Fist materia and a top-level materia such as a DMW Materia or Ultima. I'm not sure though.
Hell magics are nice to have, but there's no point in having all of them. For bosses use Hell Blizzaga, for normal enemies use Hell Thundaga or Firaga. Hell magics can be done by elemental magic & a top level status materia (Death, I think)
Don't forget that while Costly Punch is the most powerful materia in CC, it's useless if you don't have high HP.
If you're awesome, you can make like a Dark Firaga Materia that gives +999% HP. I can't remember how to make it though.
Steal 99 Phoenix Pinions from Minerva (Easier than it sounds)
Fuse the materia w/ a Fat Chocobo Feather
Fuse that materia w/ 98 or such Phoenix Pinions
+999% HP!
And you can fuse that with other attribute items to make it +99 STR, +99 VIT etc.
Go hell magic! Mo, ha, ha, ha, ha! You should have seen my best setup. ^^
In response to your question about Costly Punch...
Costly Punch is the highest grade (8) Materia in the Punch class.
Upon use, it comsumes a small portion of your HP (can't recall exactly how much, sorry) and, like all other Punch Materia (except Goblin Punch), it IGNORES DEFENSE.
Costly Punch has a damage modifier of HP. Its damage output increases as your HP decreases.
However, if your HP is at 11% higher than your normal HP, Costly Punch will do 0 damage. This is hardly a drawback, as your HP is can easily be controlled.
The Materia Fusion methods to obtaining Costly Punch:
MASTER Hammer Punch + MASTER Libra = Costly Punch
Any Punch + any DMW Materia = Costly Punch
In response to your question about Hell, "Heaven", and Holy magic...
"Hell" grade Materia is a higher grade Materia of the "Dark" Materia, and they only exist for each one of the elemental types.
Their effects...
Dark Fire, Dark Fira, Dark Firaga: inflicts Poison and Silence.
Hell Firaga: inflicts Poison, Silence, Stop, and Sudden Death.
Dark Blizzard, Dark Blizzara, Dark Blizzaga: inflicts Stun.
Hell Blizzaga: inflicts Stun, Poison, Silence, Stop, and Sudden death.
Dark Thunder, Dark Thundara, Dark Thundaga: inflicts Poison and Silence.
Hell Thundaga: inflicts Poison and Silence.
There is also a Tri-Thundaga that, in addition to being a very powerful attack with wide range, inflicts Poison, Silence, Stop, and Sudden Death.
And, finally, there are no "Heaven" or "Holy" magic in Crisis Core (as far as I know).
I hope that answers some of your questions.
P.S. my first post! ;/
